Project Access NOW is a broad based community initiative the builds access to health care
for low-income and uninsured people. They are based in the Portland/Vancouver area, however they
have links to Project Assess areas around the country. They can put you in touch with local resources as well.

WA State Disabled Parking Permit. Here is the official form in pdf format. You can complete the form online and then print it off.
Bring it in with you to the office for the signature.
*Please note: we follow the definitions of disability for this permit as
outlined on the form and will not authorize a permit if you do not fit the Washington State established
criteria. No Exceptions!
