Aberdeen Primary Care is closing (last day for appointments is Friday May 20, 2011). We are relocating to the Olympia area with a different focus of care.

Please arrange for a new provider. You may download the release of information below, fill it out and mail, fax or bring it to the office. We will release your records (on CD) to your new provider.

If you will need medication refills prior to seeing your new provider, please arrange for them now. We will not be able to refill medication after 5/20/2011.
